The Emergency Medical Services Department is seeking an energetic and forward- thinking professional to join our expanding Vulnerable Patient Navigator (VPN) Program where collaboration is valued and excellence is promoted. POSITION SUMMARY As a member of the Essex-Windsor EMS management team, and reporting to the Deputy Chief of Professional Standards, and working in a collegial/collaborative relationship with the Captains of Professional Standards, the Clinical Practice Manager is responsible for the advancement of Community Paramedic knowledge and professional practice to ensure evidence-based, individualized care of patients and families. Providing clinical direction, leadership, supervision, management, and strategic vision to a team of eight (8) full-time Vulnerable Patient Navigators. The Clinical Practice Manager will lead and supervise all clinical aspects within the VPN team in supporting and promoting the VPN mandate of Facilitating, Advocating, Coordinating and Educating our patients. The Clinical Practice Manager will be responsible for the daily clinical oversight of the VPN Program in order to meet the needs of patients and staff, and to support the mission and vision of the Program. This position leads and consistently executes on clinical work standards, policies, goals, and strategies for improving the VPN Program. The Clinical Practice Manager will monitor, supervise, direct, and consult with the VPN Team members at every stage of a patient’s care path, in order to ensure effective and efficient community paramedicine delivery to our communities.

Qualifications

Degree in Nursing and successful completion of an approved Generalist Nurse Practitioner Program or equivalent; • Nurse Practitioner designation and registration with CNO, in good standing; • Experience in clinical nursing, and interprofessional leadership and education; • 2+ years nursing supervisory experience; • In depth knowledge of the healthcare industry and Community Paramedicine Programs; • Demonstration of strong leadership/management/supervisory and coaching skills in the development of team members and support staff; • Proficient in business writing, editing, research and records management; #31-21 May 26, 2021 • Excellent organizational, comprehension, and concentration skills; • Strong communication skills are essential as is the ability to work effectively with community partners, the general public and staff; demonstrated negotiation and conflict resolution skills; • Demonstrated ability to work in a collaborative practice model with interprofessional teams; • Demonstrated ability to use initiative and work autonomously utilizing critical thinking skills, advance decision-making, problem recognition and problem-solving skills; • Well developed computer skills, including MS Office • CPR certified; • Physically able to perform medium to heavy lifting during home visits; • Valid class “G” Ontario driver’s license. • The successful candidate will be required to provide a satisfactory Vulnerable Sector Criminal Records Search as well as a satisfactory driver’s abstract.
