Job Listing | Community Paramedicine Patient Navigator

Reporting to the Commander, Community Paramedicine, the Community Paramedicine Patient Navigator is responsible for collaborating with allied health care partners to identify community health and social resources, establishing referral pathways, building networks within the community, creating and maintaining a database of community services. The Patient Navigator will support patient navigation for complex clients by linking clients with health, social and community services and programs. Anticipated contract end date - March 31, 2026.
